Philosophy of Care
Pilgrim Manor Skilled Nursing & Rehabilitation Center provides a therapeutic and caring environment, with a primary clinical goal of assisting each resident to achieve increased self-sufficiency, enhanced self-esteem, and a renewed sense of well-being. Our staff is uniquely trained to offer care for individuals with Alzheimer's Disease, dementia, and behavioral problems.

Rehabilitation Services
Pilgrim offers 7 day per week comprehensive skilled rehabilitative services for their residents. Our specially trained team of physical, occupational and speech therapists offer full support for individuals ranging from those requiring short term, sub-acute therapy services to those residents with medically & psychologically complex conditions.

Alzheimer's Program
Our center's Alzheimer's Unit is called Bradford Place. It is a secured, Joint Commission accredited Alzheimer's Unit consisting of a dedicated team to care for patients throughout all stages of their disease.

Our dedicated team includes board-certified Neurologists and a staff Psychiatrist, a Nursing Unit Manager, a Rehabilitative Program Manager, Therapeutic Recreation and an interdisciplinary clinical team of professionals.

Our Behavioral Program:
Pilgrim Manor also offers a secured unit that provides a variety of skilled services to help manage patients with behavioral problems. Our clinical team is comprised of board-certified on-site Neurologists, a board certified staff Psychiatrist, a Nursing Unit Manager, a Rehabilitative Program Manager, Social Services and an interdisciplinary clinical team of professionals.

    • Specialized nutritional, rehabilitative, therapeutic and social services offered 7 days per week

Our Rehabilitation Program Offers:
    • Specialize in rehabilitating residents often overlooked due to their
      mental or behavioral status
    • We are Medicare, Medicaid, and JCAHO certified
    • We offer weekly orthopedic rounds for rehabilitation patients
    • We offer a specialized Walking Program for residents

Specialized Offerings:
    • Certified, JCAHO Accredited, Special Care Alzheimer's Unit
    • Neurological Rounds with Medical Director, Nurse Program Mgr.,
      House Neurologist & Psychiatrist

Other Services:
    • Weekly skin rounds performed by Medical Director and Assistant Director of Nursing
