MISSION:
We at TLC Home Health Care Services realize that when someone is unfortunately unable to be independent, their family members are in crisis. Our goal, therefore, is to provide health care services to our clients that will enable them to be more self-sufficient while enhancing their well being, dignity and quality of life.

TLC works with the client, the family, primary care giver and health care professional to tailor a plan that enables the client to set his or her goals. The plan offers affordable and flexible services that allow the client to benefit from assisted living in their own homes and to continue to make their own lifestyle choices. Family members are involved but not burdened or overwhelmed.

Our caring services are available to elderly people, postoperative patients, physically and mentally challenged persons and those experiencing chronic illness or injuries.

SERVICES:
At the beginning of each case, TLC spends considerable time with the client, the family, medical and other professionals to create a customized service plan. On an ongoing basis, our evaluation and feedback system enables us to adjust services to meet the client’s changing needs. Depending on the scope of the client’s needs, TLC provides the following types of services:

Certified Nurse Assistant to provide care relating to personal hygiene, nutrition, pain management, incontinence, ambulation, exercise, transportation and household chores.

Companion Care to provide emotional support and assist with shopping, meal preparation, recreation and other daily activities.

Transportation Service that offers insured coverage to our CNA’s when driving client to any chosen location.

Case Management Services such as coordinating with health care professionals, arranging for medical equipment, monitoring dietary plans, maintaining records, prescriptions, supplies, and assist with bill paying, insurance claims and other needs.

CERTIFIED NURSE ASSISTANTS:
TLC health care providers are schooled, trained and certified by the State of Massachusetts as CNA’s (Certified Nurse’s Assistant). They are insured and have ongoing supervision. Medical care performed by our CNA’s are under the guidance and direction of medical professionals

Before allowing CNA’s to begin work with a client, we perform a reference check, CORI report clearance, verification of CNA state certification, driver’s license, liability insurance and physical examination.

The CNA’s are available to provide services up to 24 hours a day, seven days a week. TLC management work closely with the client in continually offering a plan of care that will be meet his or her needs. Most of all, we are proud of the fact that reports from our clients indicate that TLC providers demonstrate great compassion in providing every aspect of care.

EASY REFERRAL PROCESS:
Simply call our office any time, 24 hours a day, seven days a week. We will be pleased to arrange an initial visit with you at your convenience. At that time, we will discuss needs, assessment and appropriate fees for either insurance or private pay.

PRESIDENT AND FOUNDER OF TLC HOME HEALTH CARE SERVICES
Kathleen has extensive hands-on and management experience in home health care and hospitality fields. Her technical certification from the State of Massachusetts includes a Certified Nurse’s Assistant, Certified Physical Therapy Aide and Certified Rehabilitation Aide. Her continuing education includes courses in Gerontology, Psychology and Sociology. Kathleen is highly dedicated and committed to the TLC mission and is an inspiring force to providers as well as clients. She has resided in Hyannis, MA for over 20 years and offers excellent work references.
